Purpose: to help you understand whether your symptoms may be related to tension of the dura mater, the membrane that surrounds the brain and spinal cord.

For each question, indicate the frequency or intensity of the symptom:
0 = Never / Absent | 1 = Rarely | 2 = Sometimes | 3 = Often / Always

1. Do you suffer from headaches, especially in the back of the head or occipital area? *
2. Does the headache worsen when you cough, sneeze, or bend forward? *
3. Do you experience neck stiffness or pain, especially in the morning? *
4. Do you feel pain or tension in the shoulders or upper back? *
5. Do you have pain that radiates down the arms or along the spine? *
6. Do you feel pain or heaviness in the lower back or sacral area, especially after prolonged sitting? *
7. Do you experience pain or fatigue in the legs without an apparent cause? *
